Answers from Dr. Kaiser Question: I have a CD4 count of 670 with 30 percent CD4's . My doctor tells me that these are good numbers. However I suffer from leukoplakia and fatigue on an almost constant basis.I have now increased my K-PAX vitamins to the double strength dosage, and I also take 800mg of acyclovir daily which seems to help. Is there anything else you can suggest that would help beyond this. You are a wonderful doctor who has given me light at the end of a sometimes dark tunnel. Many thanks! Pierre Dr. Kaiser replies: Dear Pierre, The kind of fatigue and immune system stress that you describe (leukoplakia), despite having a high CD4 cell count, is almost always due to one of two reasons. First, it is possible that you have a secondary infection that has not been identified and is currently untreated. Examples of this could include intestinal parasites, syphilis, or hepatitis. If your liver tests are normal, that would make hepatitis unlikely so you should ask your doctor to check you for the other two conditions: intestinal parasites and syphilis. Next, it is very possible that you are walking around with one or more hormone deficiencies that are critical for supporting energy production and the functioning of your immune system. These important hormones include testosterone, DHEA, and thyroid and one or more of them could be low and a direct cause of your symptoms. Therefore, make sure that your diet includes a healthy amount of protein, continue taking the double-strength dosage of K-PAX vitamins, and get tested for the above possible factors as soon as possible. I will also be posting a detailed paper on the treatment of hormone deficiencies on my website in the next couple of days.
