Question:

I am a 42 year old woman who was diagnosed 6 months ago (4/2003) with HIV.   I'm trying to decide when to start on meds.

My doctor has suggested starting early on Epivir (3TC) and Viread (tenofovir) because there has been a lot of success when treating patients early in the disease. It is most likely that I was infected two years ago.

My CD4 cells are 661, viral load 19,000. I also have HPV and a history of cervical dysplasia.  What is your opinion?

Dr. Kaiser replies:

Of course I would need to have all the information that would come form a thorough medical history and physical exam before being able to make firm recommendations on your situation.

However, I can make a few general comments that might be helpful to you.

First, you state that you were probably exposed to HIV at least two years ago.  Therefore, you are no longer in the acute phase of the infection that requires early intervention with antiviral medications.  That phase lasts for the first six months after exposure and early treatment with antiviral meds is believed to be especially beneficial during that time.

Once you past this point, as long as your viral load is relatively low (less than 55,000 copies), and your CD4 cell count is relatively high (greater than 350 cells), I always recommend utilizing an aggressive program of natural therapies to help delay the need for antiviral medications as long as possible.  A program of aggressive natural therapies includes dietary changes, high-dose vitamin supplements, regular exercise, adequate rest, and effective stress management.  Good luck!
