Important Patient InfoAs of December 30th, 2009, Dr. Kaiser will be taking a sabbatical and his medical practice will be closed for an unspecified period of time. If you did not receive a notification letter in the mail, you can receive a copy of it today. It would also be a good idea to make sure that our office has your current contact information should we desire to contact you again in the future.
It is your right to obtain a copy of your medical records. Your records will be forwarded to you by mail. The following charges will be billed to your credit card in order to cover the administrative and mailing costs associated with this procedure. He will also be posting regular messages to website viewers as part of a new blog feature. If you’d like to be informed of any new changes to his situation, you can also sign up for his E-Newsletter which will be sent out periodically (see right column for sign up instructions). Dr. Kaiser has provided a referral list of highly respected health care practitioners in the Bay Area in order to best meet the future health needs of his clients.
